Prerequisites: Set the CREATE REVIEW AT NEW HIRE switch to 1–7 on H0CUS if you want this screen automatically created for all new hires.
Key Fields: AGENCY, EMPLOYEE SSN, REVIEW DT
Step Action
1 Access H61U1 using AGENCY and EMPLOYEE SSN.
2 Enter R or I in the SUBCOMMAND field.
3 Enter the following fields as required:
AGENCY (Key)
EMPLOYEE SSN (Key)
PERFORMANCE PLAN (Optional)
Enter Y to indicate the supervisor has completed a performance plan on the employee.
DATE REC (Optional)
Enter the date the Performance Plan was completed or when Human Resources received the plan. Required if Performance Plan is Y.
REVIEW DT (Key)
EVAL/REVIEW #1 SSN (Optional)
The employee SSN (with two leading zeroes) of the supervisor who will complete this review.
EVAL/REVIEW #2 SSN (Optional)
The employee SSN (with two leading zeroes) of the second supervisor who will complete this review.

Note: The evaluator’s name will display on Report 559, which will also indicate when the review is due.

Note: If you want the system to verify that an evaluator is allowed to do a review, set the VERIFY SSN IS AN EVALUATOR switch to Y on H0CUS. Then, enter Y in the ALLOWED TO REVIEW field on H0BUN for each employee you want to be eligible to complete evaluations.

Note: In the second part of this field, an agency may enter a five-character alphanumeric agency-defined descriptor associated with the employee’s performance appraisal.

EXCLUDE FROM REPORT INDICATOR (Optional)
Y Exclude this scheduled review from Report 559.

Note: If the employee has been terminated or is on LWOP, this review will be automatically excluded from Report 559.

NEXT EVALUATION TYPE (Optional)
Determines the date of the next review record.

O Other
P Probationary
S Special
1 One month
2 Two months
3 Three months
4 Four months
5 Five months
6 Six months
7 Twelve months
8 Annual

Note: If annual is used, the next review date is calculated based on the employment date (next anniversary date from the current date). All other values calculate the next review date from the review date displayed.

NEXT REVIEW DATE (Optional)
The date of the employee’s next performance evaluation. This field is required when next evaluation type is M, O, P, or S. If using number of months for next evaluation type, the system calculates the next review date by adding the number of months to the current review date. If using annual for next evaluation type, then the next review date is calculated as the employee’s next anniversary date.
