Report 559 Performance Appraisal
This report displays details regarding an employee's performance appraisal. The due date, how many days it is overdue, if applicable, and who is going to do the evaluation.

| Additional Notes | There are five options when requesting this report that affect the sort sequence and page breaks:
When sort option 3, 4 or 5 is requested, Evauator #1 or #2 will display between the report header and organization code. When sort option 5 is requested, only those employees with an Evaluator #2 defined will appear on the report. An asterisk(*) will appear on the last page of the report or on the bottom of a page depending on the sort selected and will indicate EMPLOYEES FIRST EVALUATION. The asterisk(*) will also appear by the new hire name. |
Report Headings
- AS OF HNK.PMEDT
- Displays the end date of the request period.
- ORGANIZATION CODE: HNH.ORGCODE
- Displays the employee's organization code.
- EMPLOYEE NAME HID.RPTNAME
- Displays the employee's name.
- SSN H61.EMPLOYEENO
- Displays the employee's masked SSN (i.e. XXX-XX-NNNN).
- JOB CLASS TITLE H70.JCTCTLE
- Displays the job title of the employee's assigned job class code.
- FUNC JC TITLE HML.FCTITLE
- Displays the functional title of the employee's assigned functional job class code (if applicable).
- PERF PLAN H61.PERFPLAN
- Displays the 'Y' or 'blank' to indicate when a performance plan has been provided.
- EVAL TYPE H61.TYPEEVALUA
- Displays the type of evaluation due for the employee based on ENCODE/DECODE 8I0.
- EVALUATION FROM DERIVED
- Displays either the hire date for the employees evaluation or for subsequent evaluations the prior evaluation end date plus one (based on prior review date).
- EVALUATION TO H61.HISTORY DT
- Displays the review date plus one day from H61U1 that is currently overdue for the employee.
- DAYS OVERDUE H0I.CURPROCDT – H61.HISTORYDT
- Displays the number of days the evaluation is overdue.
- EVALUATOR #1 DERIVED H61.EVALSSN1
- Displays the first person assigned to evaluate the employee.
- EVALUATOR #2 DERIVED H61.EVALSSN2
- Displays the second person assigned to evaluate the employee, when applicable.
