AGENCY NUMBER (Key)

APPLICANT NUMBER (Key)

Enter the applicant identification number from HMFU1 (Applicant Identification).

NAME (Display)

The applicant's name.

EMPLOYMENT PREFERENC (Optional)

Indicates the employment preference the applicant is requesting. Press F2 twice for values (Decode Table 813).

OTHER THAN 8 TO 5 (Optional)

Indicates whether the applicant is willing to work other than an 8-to-5 schedule.

OTHER THAN MONDAY – FRIDAY (Optional)

Indicates whether the applicant is willing to work other than a Monday–Friday work week.

PRIOR SERVICE (Required)

Enter a value to indicate whether this applicant has prior state service.

I-9 STATUS (Required)

Enter a value to indicate whether this applicant has met the requirements for his or her I-9 form.

WILLING TO TRAVEL (Optional)

Indicates whether the applicant is willing to travel.

TRAVEL PERCENTAGE (Optional)

Indicates the percentage of time the applicant is willing to travel.

DRIVER’S LICENSE NBR (Optional)

The applicant’s driver’s license number.

STATE (Optional)

The applicant’s driver’s license state. Press F2 twice for values (Decode Table 820).

CLASS (Optional)

The applicant’s driver’s license class.

GEOGRAPHIC PREFERENCE (Optional)

The applicant’s relocation preferences. Press F2 twice for values (Decode Table 822). This is an agency-defined decode table. If there is no agency table, any value of up to 16 digits may be entered.

CRIMINAL HISTORY (Optional)

Indicates whether the applicant has a criminal history.

CLASSES REQUIRED (Optional)

Indicates where an applicant is required to attend a class before hire. Press F2 twice for values (Decode Table 828) – this is an agency-defined decode table and must be set up in order to use this field.

EDUCATION LVL (Optional)

Indicates the applicant’s level of education. Press F2 twice for values (Decode Table 803).

TRANSCRIPT AVAIL (Optional)

Indicates whether a transcript was available at the time the application was received.

CONCENTRATION (Optional)

The applicant’s area of educational concentration. Press F2 twice for values (Decode Table 854).

LICENSE/CERT (Optional)

Any license or certification that was awarded. Press F2 twice for values (Decode Table 842).

LIC/CERT ISSUED BY (Optional)

The state in which the license/certification was awarded. Press F2 twice for values (Decode Table 820).

NUMBER (Optional)

The number associated with the license/certification that was awarded.

EXPIRATION (Optional)

The month and year that the license/certification expires.

LANGUAGES (Optional)

The two-character code that indicates the applicant’s additional language ability. Press F2 twice for values (Decode Table 824).

VISA STATUS (Optional)

Indicates the applicant’s visa status. Press F2 twice for values (Decode Table 877).

VISA EXP DT (Optional)

The applicant’s citizenship document expiration date.

RELATIVES AT AGENCY (Optional)

Indicates if the applicant has relatives working at the agency.

REFERRAL SOURCE (Optional)

The source where the applicant was made aware of the job opening. Press F2 twice for values (Decode Table 806).
