HMFU1 – Establishing Applicant Identification Information

Prerequisites: None
Key Fields: AGENCY NUMBER, APPLICANT NUMBER

Note: Use this screen to set up basic applicant information, such as name, address, and telephone number.

Step Action
1 Access HMFU1.
2 Enter I in the SUBCOMMAND field.
3 Enter the following fields as required:
AGENCY NUMBER (Key)
APPLICANT NUMBER (Key)
Enter a unique number to identify the applicant. This number can be the applicant’s SSN, with two leading zeros. If this field is left blank, the system assigns an applicant number.
LAST NAME (Required)

Note: If the name has a suffix, put it in the LAST NAME field preceded by a space. For example, John C Smith Jr or William E Harris III (no periods or commas).

FIRST NAME (Required)
MIDDLE NAME (Optional)
OTHER 1 (Optional)
Any previous name or alias used by the applicant.
OTHER 2 (Optional)
Any previous name or alias used by the applicant.
SHORTHAND (Optional)
Indicates the required shorthand speed for the posting.
ADDRESS LINE 1 (Required)
ADDRESS LINE 2 (Optional)
CITY (Required)
STATE (Required)
Enter a value to indicate the applicant’s state of residency. This field defaults to TX. Press F2 twice for values (Decode Table 820).
ZIP (Required)
COUNTRY (Optional)
This field defaults to USA.
PHONE (Optional)
The applicant’s daytime telephone number.
EXT (Optional)
The applicant’s daytime telephone extension number.
ALTERNATE (Optional)
The applicant’s alternate daytime telephone number.
EXT (Optional)
The applicant’s alternate daytime telephone extension number.
EMAIL (Optional)
The applicant’s personal email address.
APPLICANT DATE (MMDDYY) (Required)
Enter the date the applicant completed the application. This field defaults to the system date.
APPLICATION TYPE (Required)
Enter a value to indicate the type of applicant received.

GENDER (Optional)
The applicant’s gender.

Enter a value to indicate the applicant’s ethnicity.

7 Two or more races (MULTI)
BIRTH DATE (MMDDYYYY) (Optional-until time of hire on HMGU2)
Identifies the applicant’s birthdate.
PLACE OF BIRTH (Optional)
Specifies the city in which the applicant was born.
STATE (Optional)
Specifies the state in which the applicant was born.
COUNTRY (Optional)
Specifies the country in which the applicant was born.
EMPLOYEE IND (Optional)
Indicates whether the applicant is a current or former state employee.

TERM DATE (MMDDYYYY) (Optional)
Indicates the date the applicant was terminated.
LOCATION (Optional)
The location code from HNJU1 that identifies the location where the applicant was last employed by the current agency.
POSTING (Optional)
Identifies the posting number the applicant has applied for.
POSTING DATE (Optional)
Indicates the date the position was posted (the posting's effective date).
RECEIVED DATE (Optional)
Indicates date the application was received. If no date is entered, this field defaults to the system date.
REFERRAL SOURCE (Optional)
Indicates the applicant's referral source. Press F2 twice for values (Decode Table 806).
