The function name associated with the EEO-4 function code.

CONTROL NO. (Required)

Enter the unique identification number assigned to the jurisdiction by the EEOC.

NAME (Required)

Enter the name of the agency contact for EEO-4 information.

TITLE (Optional)

The title of the agency EEO contact.

ADDRESS/CITY/STATE/ZIP (Required)

Enter the address of the EEO-4 contact.

PHONE (Optional)

The phone number of the authorized official.

EXT (Optional)

The extension of the authorized official, if one exists.
