2 Enter R in the SUBCOMMAND field.
3 Enter the following fields as required:
AGENCY (Key)
NAME (Required)
Enter the name of the agency official authorized to represent the agency in EEO matters.
TITLE (Optional)
The title of the agency official who certifies the EEO-4 report.
PHONE NUMBER (Optional)
The phone number of the authorized official.
EXT (Optional)
The extension of the authorized official, if one exists.
4 Press Enter to process the transaction.
