Note 11 –Donations from Federal Assistance

Each agency must complete Note 11 either by entering the donations the agency received from federal assistance or an agency may select the Not applicable field.

Select the type of donation from the ADonations from Federal Assistance – Donation Type drop-down menu.

Enter the dollar value in the Fair Market Value at Time of Donation field.

The agency may enter additional details about the donation in the Comments field or leave the field blank.

Click Save once the statement is verified.
