Reversal Request Form

When an agency is unable to access the Direct Deposits – Reversals and Reclamations (DDEP) web application, the Direct Deposit Reversal Request form (74-191) form new window must be submitted to the Comptroller’s Payment Services section. The agency must provide an explanation why it is unable to access DDEP.

Complete all sections of the form. Incomplete forms will not be processed.

The same reversal deadlines set by Nacha rules still apply when submitting Form 74-191. Agencies must contact Payment Services to confirm receipt of Form 74-191, which may be faxed or submitted via an encrypted email.

Reversal requests submitted on Form 74-191 and processed by Payment Services are included the agencies’ reversal reports.
