Form 74-191

The Direct Deposit Reversal Request form (74-191) is a dual-purpose form used to request the recall of a direct deposit payment issued in error. Form 74-191 is used to:

• Reverse payroll or annuity payments or
• Reverse vendor or travel payments

You must complete all sections of the form. For the Type of Payment section, complete either Payroll/Annuity or Vendor/Travel as applicable. Incomplete forms are not processed.

Processing (reversal) deadlines are stated at the top of the form, as shown in the screen shot below.
