Claim Forms

All claims against the state must be submitted in the format and method prescribed by the Comptroller’s office. The claims must be prepared to provide for all of the following:

  • Authorization of the head of the office or other individual responsible for the expenditure
  • The appropriation against which the disbursement is to be charged
  • Information required by the Comptroller’s office rules
  • Proof the claim was presented to the state within the period of limitation provided by Texas Civil Practice and Remedies Code §16.051 or other applicable statute
  • Other appropriate matters
