This report lists all retirees and employees on FMLA on ERS Online for whom an agency is contributing toward insurance. This report is run by USPS Production Support after the file is received from ERS. This report updates the HNKBA – "RPT 575 LAST PRCSD ON" field with the date that this report was last run.

Report Headings
- BILLING PERIOD BEGIN DATE
- Displays the beginning date of the billing period.
- EMP SSN
- Displays the retiree's SSN.
- EMPLOYEE NAME
- Displays the employee/retiree's name.
- FUND NO
- Displays the fund number reported for the employee/retiree's fund.
- STATE CONTRIBUTION
- Displays the state's contribution for the employee/retiree's insurance, including the Basic Life premium.
- SKIP CONTRIBUTION
- Displays the state's contribution for SKIP for the employee/retiree's insurance.
- RETIREE/FMLA
- Displays if the person is a retiree or an employee on FMLA.
- FUND TOTAL
- Displays the total contribution for the FUND NO.
