ACA 1094-C and 1095-C Reporting Requirements

Update Contact Information

All USPS and CAPPS Central agencies are required to submit this form to the Comptroller’s office by Dec. 31, 2020 to ensure that the information for Forms 1095-C and 1094-C is current. This information will appear on Forms 1094-C and 1095-C, except the contact name, which only appears on Form 1094-C.

Receipt Confirmation

When the agency form has been received, the Comptroller’s office will email the agency contact a receipt confirmation. Please allow five days for confirmation.
