All USPS and CAPPS Central agencies are required to submit this form to the Comptroller’s office by Dec. 31, 2018 to ensure that the information for Forms 1095-C and 1094-C is current. This information will appear on Forms 1094-C and 1095-C, except the contact name, which only appears on Form 1094-C.
When the agency form has been received, the Comptroller’s office will email the agency contact a receipt confirmation. Please allow five days for confirmation.
